Family Application for Child & Adolescent Mental Health Division (CAMHD) Services via the Hawaiʻi State Department of Health: To start services with CAMHD, you will first need to sign this consent form. This form allows your child to receive a mental health evaluation to determine if they have a diagnosis. Based on the information from this assessment, your child may fit the criteria of the youth served at CAMHD, which would then qualify them to receive treatment.
Online Application: Using the map below, click on the area where the child/youth’s parent / caregiver / guardian lives to submit an application to the Family Guidance Center serving that region. The application requires a valid email address. To learn more about CAMHD’s application process, including eligibility, please visit their website.
